Background Information:

So I found out that I was pregnant a week ago today while attending a postpartum OBGYN checkup (I am three months postpartum). During the checkup, I brought up that I have not had my period yet but have been spotting for 26 days - I was then tested for pregnancy, std’s, and my thyroid. Everything came out normal except the pregnancy test - which was positive. My HCG count was 13,000 but could not date the length due to not having a LMP to date back to. Two days later I went to the Emergency Department believing I was having an ectopic pregnancy due to severe pain and continued spotting. An ectopic was ruled out following a transvaginal ultrasound as there was an intrauterine gestational sac visible and measuring 6w1d and an HCG count of 19,650 (not doubling but raising). However, no fetal pole or yolk sac was visible which was “highly suggestive as a failed first-trimester pregnancy via missed abortion (med term for missed miscarriage) or blighted ovum. I followed up with my OBGYN three days later and the transvaginal ultrasound had not changed (had not changed size, with no fetal pole or yolk sac visible).. however, the sac did drop lower into my uterus as it was sitting high. - but my HCG rose again to 30,100 (again, not doubling but not dropping).

Symptoms throughout this whole process:

Severe nausea accompanied by sharp stomach pains (that I did not have with my prior pregnancy) to the point I can not hold down solids or liquids. Severe lightheadedness upon standing (I faint almost 50% of the time when going from laying/sitting to standing), severe lower back pain and lower abdominal pain (like the equivalent to my labor contractions??). Spotting for like 33 days!!! (But 0 heavy bleeding) It started out like a lighter period, red in color, tiny clots, then went to a deeper red (reminded me of my lochia), (back and forth between those colors for a while) then to pink, then light brown, then back to pink (also came out in a like a stream mixed with clear fluid and reminded me of water breaking but wayyy less). No pregnancy symptoms (other than nausea - prior pregnancy, I had ALL of them by this point).

Question: Has anyone else gone through this or anything close to this??? (Recap)

- 6w 3d with visible intrauterine gestational sac with no fetal pole or yolk sac (on transvaginal ultrasound)

- Raising but not doubling HCG (not doubling in a 72hr window)

- consistent spotting (way past conception - starting out like a period with tiny clots - so its not implantation) but no heavy bleeding

- severe contraction like back and abdominal pain, and severe nausea with no other pregnancy symptoms?

- fainting. A lot.

Reason I’m asking:

I live in a strict “no abortion” state where medical intervention is illegal unless all “medical emergency/medical necessity” criteria is met - and the fact that I’m not heavily bleeding or dropping in HCG does not meet that criteria. But my body also is not taking the steps to expel this (said to be) non-viable pregnancy. The pain and symptoms are physical torture and the reminder of this non-viable pregnancy being stuck in my body is psychological torture. I wake up everyday knowing that I am carrying my dead baby and now I am questioning everything just to make it make sense???? Maybe it is viable but developing at a slower rate? Maybe the doctors are right and it is a MMC? I’m just at a complete loss and any answers/personal stories/ etc are highly appreciated!!! I’m so sorry you’re going through this. I remember feeling very similarly with my first MC - questioning everything even though ALL of the signs (and the diagnosis) pointed to an MMC. I was in the ER and even called my doctors office multiple times from the ER to see if they agreed with the OB in the ER. I suppose this is the denial phase of grief.

Unfortunately it does sound like you have an MMC. I’m so sorry you live in a state where you’re not able to receive proper care. It’s terrible that your life has to be in jeopardy before you are able to medically manage a clearly non viable pregnancy. If you want to medically manage the pregnancy talk to your doctor. There are ways you can get the meds you need via mail. You can also travel to another state to receive the care you need. Again I’m so sorry you have to go through this - no woman should be forced to endure what you’re going through because a bunch of men in charge or women who have never been through this don’t understand women’s health issues. There is no harm in waiting and making completely sure it isnt a viable pregnancy. I had a MMC at my 9wks scan, baby had stopped developing at 6wks 4 days and no heartbeat. I've only just passed everything a couple of days ago at what I was supposed to be 12wks. I didn't want to take the pills or to have a d&c.

I just want to get this over with and move forward :( I can’t even take care of my 3 month old daughter because I can not stand and hold her because of constant fainting - and getting up is excruciating with the back and lower abdominal contractions. I also suffer from severe PPD (to the point I had to stop breastfeeding to get on medication) and now I have to be off those meds for the pregnancy- adding on the preexisting depression, PPD, and now grief. My S/O is taking way too much time off of work to help with her care during the day. My 3mo olds care is in jeopardy because I can not get this mmc taken care of or get a straight diagnosis even. “Possible MMC” but without heavy bleeding, dropping HCG, or being (At minimum) 8w from “LMP” (which I never got so that’s impossible to determine if I am having a MMC/stopped fetal growth) - I can not get a direct diagnosis.

1) typically by 10,000 hcg they would want to see cardiac activity (and thus also a fetal pole and yolk sac)

2) hcg can continue to rise despite a non viable pregnancy especially when the sac is still attached it will still increase hcg

3) spotting and bleeding can be a few weeks before the actual physical outward mc process starts

4) you could do one more follow up scan a week later to confirm nothing has changed

5) the hcg will start to drop...

the hard thing is you have no idea on timing so its a bit hard to know how many weeks post conception you are - it can take a few days up to 4+ weeks for the mc to take place

if you are 3 months pp, it would take at least a month or so for your hcg to come down and your cycle to reset?? so at the very most youd be 8 weeks? measuring 6? So it could be two weeks since development stopped??

your hcg is still going up but it will peak out and then turn down - its just hard to say how long it will take...

what exactly is the criteria for being able to be perscribed misoprotol in your state?

typically there is general criteria for doctors being able to 100% classify a mc - based on either gestational sac size (without growth inside), or two scans that dont show development, an hcg of a certain number without cardiac activity, etc ...

that severe lower back pain can definitely happen as your body realizes the mc ...

the fainting should be something your doctor is concerned with - did they run other blood work? At the very least I guess be sure to stay hydrated - it sounds like you mayve havent been able to eat much so maybe thats at play also?

My state currently uses vague language up to interpretation of prosecutors because of an uproar after releasing a bill stating in blunt language that inducing/aiding in/or supplying anything to induce an abortion for an ectopic pregnancy would be a Class A Felony. - So instead they changed it to an abortion/medical intervention being allowed only for “medical emergencies” (this is interpreted typically as “life or death”/to save a mothers life) - basically like a rupturing tube/internal bleeding/sepsis. So as of now, I’m not experiencing a “medical emergency” and must wait for it to naturally come (which even with the contraction like paid in my lower abdomen and lower back for the past 3 days and spotting for over 30 days - its still not making its way out 🥲)

I had contractions from 11 am to 3 am ... then again a little bit the following day....

When I went in for the scan it had gotten into my cervix and they were able to remove the sac in the office...

perhaps you can have a u/s check to see if its still attached or trying to work itself out?

I had two natural mc and my second was definitely a whole different experience than the other - i had all the contractions and everything but my cervix wasnt dialating enough I think for it to be able

to pass through - i was nauseous and had isnane back pain and contractions pain ... my body did try over the course of a few days... and then my dr was able to help with the final step...

i know its so so hard and you feel limited by things outside of your control - try super hard to believe and trust in your body to do this for you - trust that it is trying... there are positions and breathing things you can try ...

do you have any pain medication or can the dr perscribe you some while youre going through this?

Since I still have not received a definitive diagnosis (because they can not until no visible yolk sac or fetal pole by 8 weeks since LMP (which I did not have so it’s impossible to date whether I am or not) - or heavy bleeding/significant drop in HCG - they legally can not prescribe any medications deemed unsafe for pregnancy. So I was told to take 1,000mg of Tylenol a day and drink water. There is no amount of Tylenol in the world that would ease this pain even slightly 😭

I believe since it did drop Sunday night/Monday morning lower into my uterus, it could possibly be stuck with my cervix not dilating (as I have experience with that already)

When I gave birth to my daughter, I was 1cm dilated at 39weeks. I received the medication directly in the vagina throughout the night of my induction and only dilated to 2cm. I was then put on Pitocin. The start of my induction to getting to where I could push took over 24 hours. They ended up having me push at 8cm (where I was at 10cm during each contraction) or I would have to go for a C-Section.

Update:

- Contraction like pain in lower pack and abdomen has become a lot stronger

- Sharp sporadic pain on my left side

- Sharp pain in my left shoulder pain

- Spotting has stopped again (its off and on quite a bit) but now I have excessive white fluid coming out every time I use the bathroom and some of it is like white and chunky???

Extremely confused because ectopic tubal pregnancy has been ruled out via 2 transvaginal ultrasounds???

Update! I have a D&C scheduled Thursday. I went in for an ultrasound today and there was still no fetal pole or yolk sac. However, the gestational sac did grow which explains my HCG rising. So this chapter will soon be coming to a close and I will be able to focus on my 3 mo old daughter.
